Scholarship for Survivors
The Scholarship for Survivors is a scholarship sponsored by Patient Advocate Foundation.
Scholarship information:
Amount of Scholarship: Average - $3000
Application Deadline: 16-April
Scholarship Requirements: Candidate must apply via postal mail and submit all required information in its entirety when returning the application. Incomplete applications will not be forwarded to the Scholarship Review Board for consideration. Candidate must write an essay on how their diagnosis has impacted their lives and their future goals. Candidate must submit a copy of an acceptance letter form the college, university, or vocational-technical school planning to attend. Candidate must also submit written documentation form your treating physician stating your medical history. Candidate must provide two letters of recommendation from non-related persons. Please ask that each reference include their names, address, and phone number within the letter. Candidate must provide an official high school and/or current college transcript. Must also complete financial form that will demonstrate financial need and include a copy of the first two pages of tax returns.
Eligibility: College freshman, College sophomore, College junior, College senior, Doctoral-level study, Master's-level study, Other postgraduate-level education
